Please call for an appointment, 330 491 3647. You will need to fill out the forms on our webpage and get a vet referral from your vet in order to swim. You can print these out from the forms, have your vet fill the referral form out and have them fax it to us or we will send a vet referral for you. We will get this information when you call us.
